The new FHP Continuity of Care Clinic (COC) officially opened for patients on Friday, Feb. 19, at 131 N. Ewing St., Unit A. Designed to support patients as they transition back to everyday life from hospital care, the COC will help keep patients healthy and out of the emergency department. Eligible patients will be contacted during their stay or within three days of discharge to schedule an appointment to be seen by the COC provider, Tyler Johnoff, CNP. Once thorough short-term follow-up care is completed with the COC, we will ensure patients are scheduled with a primary care provider and necessary specialists to manage their long-term health and wellness.

• Connecting them with available resources
• Routine monitoring to help them stay out of the ED
• Providing regular communication with a healthcare practitioner
• Education on discharge instructions, medications, treatment regimens, etc.
• Coordination of care with primary care providers and specialists for continuation of long-term care
